YOU MUST HAVE A RIDE HOME FROM YOUR PROCEDURE. UBER/LYFT WILL NOT BE CONSIDERED AN ACCEPTABLE FORM OF TRANSPORATION FROM YOUR PROCEDURE. YOUR PROCEDURE COULD BE CANCELLED WITHOUT A RESPONSIBLE RIDE HOME.
PAYMENT ARRANGMENTS ARE REQUIRED TO BE MADE PRIOR TO YOUR PROCEDURE. THE CENTER WILL ASSIST IN DISCUSSING YOUR OPTIONS. WE ASK THAT YOU BE PREPARED TO PAY FOR YOUR CO-PAYS, DEDUCTIBLES, ETC.
THE CENTER WILL FOLLOW ALL CDC GUIDELINES FOR COVID-19 INCLUDING MASKING, SOCIAL DISTANCING, ETC. WE ASK FOR YOUR CONSIDERATION BY APPRECIATING OUR CONCERN FOR PUBLIC SAFETY.
PLEASE PRINT OUT FORMS, & FILL OUT YOUR PATIENT PACKET PRIOR TO YOUR ARRIVAL AT THE CENTER. THIS WILL ASSIST IN KEEPING THE SCHEDULE ON-TIME TO THE BEST OF OUR ABILITY.
YOU MAY RECEIVE 1 OR MORE CALLS FROM THE CENTER IN THE PRECEEDING DAYS TO YOUR PROCEDURE TO DISCUSS YOUR HEALTH HISTORY, AND/OR FINANCIAL ADVISEMENT CALL.
PLEASE FOLLOW ALL INSTRUCTIONS GIVEN TO YOU FROM YOUR PHYSICIAN’S OFFICE, INCLUDING ARRIVAL TIMES. THE CENTER MAY CALL ON THE DAY OF SURGERY TO ADJUST YOUR ARRIVAL TIME IF THE SCHEDULE IS AHEAD/BEHIND SCHEDULE, TO ALLOW FOR MINIMIZED WAIT TIMES IN THE WAITING ROOM. WE WORK TO THE BEST OF OUR ABILITY TO MAINTAIN THE SCHEDULE, HOWEVER WE WILL ALWAYS TAKE THE APPROPRIATE AMOUNT OF TIME WITH EACH PATIENT/PROCEDURE.
